Did you receive an email or letter from the U.S. Coast Guard (USCG) notifying you that your application was rejected? Starting December 1, 2024, the USCG will strictly enforce rules for application completeness, leading to rejections if any part of the application is missing or incorrect. This includes applications for OUPV renewal, Six-Pack license upgrades, and 100-Ton license applications.

When an application is rejected, mariners must submit the entire package again, including corrected documents. Among the most common reasons for rejection is a missing or incomplete conviction statement using the CG-719C form.

Why Applications Get Rejected Over Conviction Statements

If you checked "yes" on any box in the Criminal Record section of the CG-719B form (e.g., convictions, drug use) but didn’t include a completed CG-719C form, your application will be rejected.

The CG-719C form is required to provide the USCG with details of all convictions that have not been previously disclosed.

Common Conviction Statement Mistakes

  1. Failure to Disclose All Convictions
    Every conviction must be disclosed, even minor ones, unless they were previously reported to the USCG.
  2. No Memo for Previously Disclosed Convictions
    If you’ve already reported a conviction in a past application, you must include a signed memo stating this. You can generate this memo for free inside your MM-SEAS account.
  3. Untruthful Statements on the CG-719B Form
    Never omit criminal history to avoid completing the CG-719C form. The USCG conducts a thorough background check, and dishonesty is considered fraud, which could lead to serious consequences.

How to Fix a Rejected Application

Step 1: Complete the CG-719C Form

Step 2: Address Previously Disclosed Convictions

If you’ve disclosed a conviction in a prior application, generate a memo inside your MM-SEAS account. This memo confirms to the USCG that your prior conviction has already been reported.

Step 3: Resubmit Your Entire Application

The USCG does not retain rejected applications or partial updates. You must resubmit your complete package, including:

  • Corrected CG-719B form
  • Completed CG-719C form or memo for previously disclosed convictions
  • Any supporting documents, like proof of fees or TWIC compliance
