On the CG-719K Section I, you check the box for what current USCG licenses you hold. If you hold more than one type check all that apply.

What do the different boxes mean on the CG719K?

Deck - This is the most common USCG license group in the US. If you hold or are applying for any USCG license as a Master, Mate, OUPV, Able Seaman or Tankerman you check Deck.

Engine - If you hold or are applying for any USCG license as a Chief Engineer, Assistant Engineer, DDE, QMED, Oiler, or Tankerman engineer you check Engine.

Food Handler - Everyone who ever has the potential to handle food should check the Food Handler box. (It's required to be on your license for some companies no matter your position on the ship)

STCW - If you hold or are applying for any STCW credentials you check STCW.

Entry-level without lookout duties is not often used and the other two are for a few mariners who have specialized USCG licenses.

MMC Held or Applying for - 100 GRT Master Inland

The USCG Medical team only accepts one big pdf file for all 10 pages.

You need to turn your medical paperwork into a pdf. We recommend using the free Adobe Scan App on your phone or you can use a scanner.

Once you have your pdf of your completed CG719K named as: Last Name, First Name, CG719K  Copy this text and then replace the Bold Below with your information.
