The USCG Medical team only accepts one big pdf file for all 10 pages.
You need to turn your medical paperwork into a pdf. We recommend using the free Adobe Scan App on your phone or you can use a scanner. The file size should not exceed 75MB.
Once you have your pdf of your completed CG719K named as: LASTNAME_FIRSTNAME_ORIGINAL or LASTNAME_FIRSTNAME_MMCREFERENCENUMBER, upload your form directly to the Coast Guard on the medical application upload page at https://nmc-asap.appsplatformportals.us/MedicalInformationUploadApp/.

We hope this made your life a little easier and if you have other questions the MM-SEAS team is always here to help!
