When I Submit My USCG Medical Form, Do I Submit It as One Big PDF File or as Many Separate Pages?

The email attachment MUST contain ALL completed pages of the CG 719K or K/E as one (1) attachment.  

Do not send a link to the attachment as it can't be accessed by the USCG's IT system and they will kick it back.

Once you have the pdf built, you can click the medical button inside of mmseas.com and we will direct you to the Coast Guard's medical upload portal.

You will receive a submission confirmation email after the submission.

Are there other ways to submit my Medical form?

If you are unable to do so electronically, the application can also be faxed to 304-433-3407.
