When I Submit My USCG Medical Form, Do I Submit It as One Big PDF File or as Many Separate Pages?

The email attachment MUST contain ALL completed pages of the CG 719K or K/E as one (1) attachment.

Once you have the pdf built you can click the medical button inside of mmseas.com and there is an option to create the medical email exactly as the USCG wants it.

It's as easy as clicking the 'at' button, adding your 10 page pdf attachment to the email, and hitting send.

Are there other ways to submit my Medical form?

If you are unable to do so electronically, the application can also be faxed to 304-433-3407.

What happens when my Medical form is rejected due to technology issues from the USCG?

If the Coast Guard has trouble with your pdf submission you will receive an email that says something like this: "Please be advised, if you are receiving this email, there will be no medical certificate application opened at the National Maritime Center."

If you get such an email, try saving the form as a pdf again and resending (a tip: make sure you save it as a pdf that is in the best format for electronic submissions), and if you still have trouble, fax it to 304-433-3407.
